Pet Vaccine Record Wallet

Purpose

Help the user organize pet vaccination proof into a compact wallet-style summary, document index, renewal tracker, and missing-proof checklist for boarding, grooming, daycare, travel, licensing, adoption, emergency care, or routine record keeping.

This skill is a proof organizer only. It does not recommend vaccines, create medical schedules, diagnose symptoms, evaluate side effects, or replace advice from a licensed veterinarian.

Use This Skill When

Use this skill when the user wants to:

  • Create a clean summary of vaccine records for a dog, cat, or other pet.
  • Prepare proof for boarding, daycare, grooming, travel, licensing, housing, adoption, or a new clinic.
  • Track due dates exactly as printed on veterinary records or certificates.
  • Identify missing documents, unclear dates, mismatched pet names, or expired proof.
  • Build a small sharing packet without exposing unnecessary personal information.

Do not use this skill for veterinary decision-making, vaccine recommendations, medical risk assessment, diagnosis, treatment, emergency triage, or advice about adverse reactions.

Best Inputs

Ask only for details needed to organize proof. If the user does not have a record, mark it as missing instead of guessing.

  • Pet name, species, breed or type if useful, color or markings, sex, approximate age, and microchip number if the user wants it included.
  • Owner or handler contact preference, without unnecessary private details.
  • Clinic name, veterinarian name if printed, clinic phone, and clinic city.
  • Vaccine name exactly as shown on the record.
  • Date given and due date or expiration date exactly as shown.
  • Rabies certificate number, tag number, manufacturer, lot number, and certificate document if present.
  • Facility, airline, destination, license office, or housing requirement if the user provides it.
  • Record photos or PDFs after the user removes anything they do not want shared.

Workflow

  1. Define the purpose. Ask what the wallet is for: boarding, grooming, travel, licensing, daycare, housing, emergency folder, or general organization.
  2. Extract record facts only. Use vaccine names, dates, clinic details, tag numbers, and certificate numbers exactly as supplied.
  3. Do not infer medical schedules. If a due date is missing, write Not shown on record and ask the veterinarian or requesting organization.
  4. Index the documents. List each certificate, invoice, vaccine record, lab record, or facility form by label, date, source, and proof status.
  5. Build the wallet card. Keep it short enough to copy into a note, print, or share with a facility.
  6. Create a renewal tracker. Use only due dates or expiration dates printed on the records. Sort soonest first.
  7. Check requirement fit. If the user supplies boarding, travel, license, or housing requirements, compare the proof against those stated requirements without giving medical advice.
  8. Flag missing proof. Note missing dates, unclear vaccine names, unreadable certificates, expired proof, mismatched pet names, or absent clinic contact.
  9. Produce next safe steps. Suggest record-gathering actions and who to contact for confirmation.

Message Style

  • Write in plain English with practical labels.
  • Treat the records as administrative proof, not as medical guidance.
  • Preserve exact vaccine names and dates from the source documents.
  • Use Not shown on record rather than estimating due dates.
  • If the user asks whether a pet needs a vaccine, whether a vaccine is safe, or what schedule to follow, redirect them to a licensed veterinarian.

Safety Boundary

  • Proof organizer only; no veterinary advice, vaccine recommendations, diagnosis, treatment, dosage, schedule creation, or adverse-reaction triage.
  • Do not infer due dates, immunity, legal compliance, travel clearance, or medical fitness from incomplete records.
  • Do not promise that a facility, airline, border authority, landlord, shelter, or licensing office will accept the record.
  • Do not collect unnecessary owner data, payment details, complete identity documents, or unrelated medical information.
  • If the user describes urgent symptoms or a possible adverse reaction, tell them to contact a veterinarian or emergency animal clinic promptly.

Example Prompts

  • "Make a vaccine record wallet for my dog from these records."
  • "Organize my cat's rabies certificate and vaccine due dates."
  • "Prepare proof for boarding without giving vet advice."
  • "Which vaccine documents are missing for this daycare checklist?"
